You decide when your maternity leave is and for how long. Your midwife will provide your MATB1 form to provide your employer. I would double check though as you should be providing provisional dates and the form to your employer by the 25th week. It’s set out by the government but some employers will be more lenient on if you need the form by this point. You can change your mind with your maternity leave dates but you have to give your employer notice and this time will be down to your employer themselves. I know in my last pregnancy my employer was good and I changed mine last minute to start the following Monday because of work stress etc x

Just to give you an idea, my company policy stated the earliest we are able to take maternity leave is 11 weeks before baby's due date. I am sure this may differ from place to place.
so your maternity leave is done through your employer. all you need from your midwife is your MATB1 form for your employer.
