I’ve just done mine and found it so much easier to do on paper, if you can print it or I requested a paper version which arrived within a few days and I sent it off in the post, old school style lol, On the government website there is also a calculator that if you put your due date in, it will tell you all the dates you need- test period etc Good luck xx
I've had to fill the form out too, a lot of it I didn't need to fill out, luckily. Don't think there was any issues as I've heard back with a different form to confirm the date I stop working and when I want the pay to start. I'm guessing I gotta send that once I've finished working.
@Rachel thank you yes I like the paper option too, I’m going to print out what I’ve got and read through. I found the calculator, that was very useful! It was just thinking back to what I was doing for work in 2023 as that feels like a long time ago!

Yeah same there was lots to skip depending on your situation, I’m self employed so luckily I didn’t have to think back! Baby brain would not have liked that xx
Just please do it as soon as you can as those forms can take a while. I sent mine after I hit 26 weeks. A week later I got a text message stating the form would be processed within the next 4 weeks. So if there are any mistakes or things they need to clarify, it may take a while to get it approved. Let me know if you have any questions x
