You can print out the paper form and after you write on the front the address which you can find on gov website. It’s free to send btw because it’s free post

You need to get your mat b 1 and your midwife should give u a form I got help filling mine out and then you give it to your UC coach once your 24 weeks pregnant and it should come into your bank after a couple of weeks hope this helps x
