Yes im finalizing mine and will be sending the first draft to my ob tomorrow before my appointment. I dont have a template but i did make it concise bullets in sections of each part of the process. I will be adding stuff i learned today in labor class too like 'No pulling on the umbilical cord', & 'Picton only if there is a need to slow down the bleeding after birth'.
