How do I get a doctors license revoked?

This Obgyn just lied on my paperwork and this was the last straw with how she’s been treating me. I’m 30 weeks pregnant and now have no idea what I’m going to do about a doctor and I’m freaking out! She’s dangerous and I already submitted a complaint online but I don’t think that’s enough for lying on medical records.
